The Guiding an Improved Dementia Experience (GUIDE) Model is a groundbreaking program funded by Medicare. Launched on July 1, 2024, the GUIDE Model is an 8-year payment initiative designed to support dementia care management, respite care, and caregiver support.
The Dememtia Connection Institute works with Lizzy Care as certified GUIDE Model participants. Thanks to the Medicare GUIDE Model, this program is completely free of charge to people enrolled in Medicare Part B and their families. Lizzy Care and Dementia Connection Institute will handle all billing with Medicare, ensuring no out-of-pocket costs, copays, or fees for participants. No Cost to Families. It’s That Simple.

Also, receive CEU credits for attending our webinars and when earning your Dementia Connection Specialist (DCS) and/or Dementia Connection Specialist Certified Trainer (DCSCT) certifications.
